Pack 374 Membership Information...

If you are interested in joining Pack 374, please contact the membership chair, Joan Wilson, at membership@cubpack374.org or (408) 460-6933.

She will be able to answer your questions and get you started with an application. Please contact her before filling out any forms. Also feel free to contact other Pack leadership.

Pack members also are assigned to a den, usually a neighborhood group of six to eight boys of the same age.

Membership Forms

	Cub Scout Application
	Adult Application This Form is only required if you wish to be a member of the Pack's Leadership Team.  Ether as a Den Leader or a member of the Committee.
	Emergency Consent Form SANTA CLARA COUNTY COUNCIL BOY SCOUTS OF AMERICA EMERGENCY CONSENT FOR MINORS
	Permanent Permission Slip Overall Pack/Den Activity Permission Slip.  Must have a valid Emergency Consent Form on record as well.
	Parent and Family Talent Survey Questionnaire that has been scientifically developed to help us find and use your talents and resources. Please take a few moments to fill this out, honestly and accurately, then drop it off with any one of the neatly dressed, uniformed Scout Leadership.
	Personal  Health and Medical Record (Form 34414B)
	Parent Commitment Form A commitment by parents and/or guardians that they understand that their Cub Scout and Pack will need their support for a positive and successful scouting experience for their boys.
	Photo and Video Release Approval to have photos and/or video footage of scouting activities included on Pack 374 Yearly Movies, Recruitment/Promotional Material, and/or the Pack web site.
	Local Tour Permit
