• Emergency Consent Form
  SANTA CLARA COUNTY COUNCIL BOY SCOUTS OF AMERICA
  EMERGENCY CONSENT FOR MINORS
     
• Permanent Permission Slip
  Overall Pack/Den Activity Permission Slip.  Must have a valid Emergency Consent Form on record as well.
   
• Parent and Family Talent Survey
  Questionnaire that has been scientifically developed to help us find and use your talents and resources. Please take a few moments to fill this out, honestly and accurately, then drop it off with any one of the neatly dressed, uniformed Scout Leadership.
   

• Personal  Health and Medical Record (Form 34414B)

• Parent Commitment Form
  A commitment by parents and/or guardians that they understand that their Cub Scout and Pack will need their support for a positive and successful scouting experience for their boys.

• Photo and Video Release
  Approval to have photos and/or video footage of scouting activities included on Pack 374 Yearly Movies, Recruitment/Promotional Material, and/or the Pack web site.

• Event Plan  Sample
  Planning document for an event. To be filled out and sent to the activities chair.

• Event Follow-up Form  Sample
  Post-event follow-up form. To be filled out and sent to the activities chair.

	FORMAT
Example DEN Code of Conduct This is the modified version of the Code of Conduct set by and agreed upon by the Webelos Scouts.  It is a modification of the Pack level Code of Conduct below
Example PACK Code of Conduct This is the modified version of the Code of Conduct that was found on the web.  It has not yet been adopted as the official Pack's Code of Conduct, however, it does give a good example and place to start.  This was modified to address formatting and then modified again to fit the Webelos above.

• Den Dues...
  Its always hard to figure a way to make sure the Cub Scouts earn their dues.  Especially if they are paid in full at the beginning of the year.  The forms below are an attempt to help the Cub Scouts learn that they should always think of doing a "Good Turn Daily" and have the responsibility to "Earn" their monthly dues.
